Requesting Your Counseling Records
To request access to or copies of your counseling records, contact Hannah Pate Sanchez at hannahpate@thefigtreecounseling.com or send a message through the secure client portal.
Please provide your name, preferred contact information, and the records or dates of service you are requesting. Please do not include sensitive clinical details in an ordinary email.
We will explain any necessary identity verification or authorization steps and arrange an appropriate method of delivery. Requests will be handled according to applicable state and federal laws, including legal timeframes, permitted fees, and any lawful limitations on access.
Contacting the Licensing Authority or Filing a Professional Complaint
Hannah Pate Sanchez, Texas LPC license number 89004, is regulated by the Texas State Board of Examiners of Professional Counselors through the Texas Behavioral Health Executive Council.
NOTICE TO CLIENTS
The Texas Behavioral Health Executive Council investigates and prosecutes professional misconduct committed by marriage and family therapists, professional counselors, psychologists, psychological associates, social workers, and licensed specialists in school psychology. Although not every complaint against or dispute with a licensee involves professional misconduct, the Executive Council will provide you with information about how to file a complaint.

Complete the Council's complaint form and submit it according to its instructions, including by email to Enforcement@bhec.texas.gov or by mail to the address above.
Tennessee Clients
Hannah Pate Sanchez is also licensed in Tennessee as a Licensed Professional Counselor with Mental Health Service Provider designation (LPC-MHSP), license number 7599. She is regulated by the Tennessee Board for Licensed Professional Counselors, Licensed Marital and Family Therapists, and Licensed Clinical Pastoral Therapists.

Filing a Consumer Complaint About Health Information Privacy
You may file a consumer complaint concerning health information privacy or medical-record issues with the Texas Attorney General's Consumer Protection Division.
Visit the Attorney General's medical privacy page and follow its link to the consumer complaint process. Follow the instructions to complete and submit your complaint.
You are not required to contact this practice before filing a complaint.
